Attn: Families of students who received their first Pfizer dose during our middle school clinics that were held on May 18 or May 24.

Dear Families,

We hope you are having a great week. We will once again partner with the Chickahominy Health District to hold vaccine clinics at all of our middle schools specifically for HCPS students who received their first dose of the Pfizer COVID-19 vaccine during our clinics that were held on May 18 or May 24. These clinics will allow these students to get their second dose. Only students who received the first dose at the prior middle school clinics and are 12 years of age or older are eligible.

The second dose vaccine clinics for eligible students will be held as follows:

- Bell Creek Middle School: Monday, June 14 from 9-11 a.m. and 4-6 p.m.
- Chickahominy Middle School: Tuesday, June 8 from 1:30-3:30 p.m. and 4-6 p.m.
- Liberty Middle School: Tuesday, June 8 from 1:30-3:30 p.m. and 4-6 p.m.
- Oak Knoll Middle School: Monday, June 14 from 9-11 a.m. and 4-6 p.m.

Similar to the first round of clinics at our middle schools, the 9-11 a.m. and 1:30-3:30 p.m. sessions are for eligible middle school students only and are not open to the general public. The 4-6 p.m. clinics are for all other HCPS middle and high school students who received their first dose at a prior middle school clinic and need their second dose of the Pfizer vaccine.

For the clinics during the school day, middle school students must return a completed permission form, which was also required for the first-dose clinics.

All students who are vaccinated after school hours (during the 4-6 p.m. clinics) must either be accompanied by:

- a parent or guardian, or
- someone 18 years old or older and have a completed parental permission form

Please note that whomever accompanies the student to the 4-6 p.m. clinic may receive their second dose of the vaccine as well, if needed.

A fact sheet on the Pfizer vaccine can be found on the FDA website. For community members who still need a first dose of the Pfizer vaccine, please contact the Health District at (804) 365-3240, and they will schedule you an appointment at the Ashland Junction clinic location.

Thank you.

Dear Families,

We hope you are having a great week. We are writing today with an important update about the COVID-19 vaccine and our schools.

Earlier this week, the U.S. Food and Drug Administration authorized the emergency use of Pfizer’s COVID-19 vaccine to be administered to children 12 through 15 years of age. The vaccine has already been authorized for children 16 years old and above.

With the expanded eligibility, we are proudly partnering with the Chickahominy Health District to hold vaccine clinics for students inside our four middle schools later this month. The vaccine clinics will be held as follows:

- Bell Creek Middle School: Monday, May 24 from 1:30-3:30 p.m. and 4-6 p.m.
- Chickahominy Middle School: Tuesday, May 18 from 1:30-3:30 p.m. and 4-6 p.m.
- Liberty Middle School: Tuesday, May 18 from 1:30-3:30 p.m. and 4-6 p.m.
- Oak Knoll Middle School: Monday, May 24 from 1:30-3:30 p.m. and 4-6 p.m.

Please note that the 1:30-3:30 p.m. clinics are for students only and are not open to the general public. The 4-6 p.m. clinics are for all other HCPS middle and high school students who are 12 years of age or older and want a vaccine, no appointment necessary and while supplies last.

For the 1:30-3:30 p.m. clinics, students must return a completed permission form to get vaccinated. All students who are vaccinated during the 4-6 p.m. event must either be accompanied by:

- a parent or guardian, or
- someone 18 years old or older and have a completed parental permission form

Whomever accompanies the student to the 4-6 p.m. clinic may receive their first dose of the vaccine as well.

To assist us in our planning efforts for these clinics, please complete this survey if you would like your student to receive the vaccine during one of these clinics.

You can find a fact sheet for the Pfizer vaccine on the FDA website. For questions about the COVID-19 vaccine, please contact the Health District at (804) 365-3240.

Thank you.

Queridas Familias,

Gracias.
